Tenant Enrollment Form Healthy Homes Program (HHP) Grant
Children's Mercy Hospital has enrolled you tenant in our Healthy Homes program which may provide updates to your unit/dwelling at no cost to you. Below is information about our program and the requested signature for completing these updates:
Children's Mercy Healthy Homes Program provides Education and Support for families residing in the Kansas
City metropolitan area. Our teams focuses on prevention strategies to aid in property upkeep that may have a
positive impact on health. The Healthy Homes Team provides a home assessment and education that includes
information on: 1) Pest control and prevention 2) Resources on home cleanliness and general upkeep, 3)
Dust/allergen management with a general indoor air quality checkup, 4) safety and injury prevention checklist,
5) And possible minor home repairs (completed at no cost to the property owner, utilizing funds from the
programs HUD Grant for income qualified families). Any updates recommended from our program for the family's home are completed with the permission of the property owner, tenant and Children's Mercy Hospital and participating housing organization for the sole purpose of improving environmental conditions in the
community.
I certify that I approve of minor home updates that are recommended through a scope of work summary, at
no cost to me, through the Healthy Homes Program at Children's Mercy Hospital.
